Our services are as varied as the people we serve. HomeLife Partners specializes in Live-In 24 Hour Care and provides short term care, hospice, respite, and hourly care. Our Caregivers have specific training in chronic illnesses including Alzheimer’s, dementia, Parkinson’s, stroke, diabetes, CHF, MS and full time bed care. After the Care Manager meets with the family, we create a detailed Care Plan based on the standards set by the National Association of Professional Geriatric Care Managers. The Care Plan outlines all the tasks our client would like provided by our Caregiver and/or Care Manager, and is a living document that will change as our relationships evolve.

Organizing medications, participating in doctors’ appointments, asking the right questions and following up with medical or insurance providers to assure all the benefits and options are understood.
Light cleaning, laundry, pet care, meal planning, cooking, clipping coupons, watering plants, sewing, taking out the garbage, answering the door and phone, supervising home maintenance and outside services.
|Mail Sorting & Bill Paying:|
Knowing what’s real and what’s advertising is becoming increasingly difficult. We can help prevent fraud by removing and shredding unsolicited mail. Managing household petty cash, assuring bills are paid in a timely manner includes writing and mailing checks, balancing the checkbooks, and reporting back to concerned family members.
Driving for entertainment, grocery and clothes shopping, doctors’ appointments, mailing letters, going to dry cleaners, buying and wrapping gifts, picking up prescriptions, going to exercise classes or physical therapy, driving to church services are all included in our hourly and Live-in rates.
|Coordinating Household Maintenance:|
Managing an older home often includes emergency plumbing repairs, fixing leaking roofs, installing grab bars and stair railings. We can recommend and manage any home repair projects and prevent small problems from becoming big ones.
Assistance with personal hygiene, toileting, changing incontinence garments, bathing, dressing, eating, medication reminders, arranging appointments, maintaining a personal calendar and exercise assistance are just a few of the personal services we offer.
Consistent email, telephone and face-to-face conferences with our clients and their families are an important part of our quality program. We don’t like surprises and neither do our clients so being proactive to alert familiy members of potential problems prevents unnecessary medical complications or household issues from escalating.
Restaurant dining, attending movies and the theater, participationg in social events such as church, bingo, club meetings, attending lectures, playing cards, letter writing and accompanying on travel adventures are all included and can keep seniors engaged and happy.
